We received a lot of entries for the first IADVL SIG Dermatopathology (IADVL Academy) Image Quiz. Please find below the answer with explanation as well as the names of those who got it correct.
Answer: Double positive (CD10 and MUM-1) Diffuse large b cell lymphoma -germinal centre type (GCB-subtype)
Histopathological findings: An elliptical excision biopsy from an erythematous nodule over right thigh was performed which showed round tumor cells infiltrating skin with narrow Grenz zone. The lymphocytes were of medium size and atypical. Nuclear atypia was evident. Plasma cells were also noted within infiltrates. Tumour cells were positive for CD20, CD45, CD10, Bcl 6, Bcl-2, MUM-1. Ki 67 proliferative index was 90%. So, it can be classified as double positive (CD10 and MUM-1) Diffuse large b cell lymphoma -germinal centre type (GCB-subtype) according to Hans algorithm with secondary cutaneous involvement. A new classification system has been proposed on the basis of gene expression profiling.
